Now that I have a new myFaves 2-year contract, what else do I need to do to qualify?

You must mail in your completed Free Flight application form, postmarked by December 31, 2007. Then, make sure you still have an active qualifying myFaves 2-year contract plan with no overdue balances as of February 29, 2008 to qualify for a free flight booking request form.

What does "active with no overdue balances" mean?

Postpaid customers with active myFaves two year plans must not have any past due balances. FlexPay customers must still be on a two year myFaves plan and must either have paid their monthly service charge or be within their 45 days grace period.

What is a booking request form?

The booking request form allows you to choose your top three choices for travel destinations and dates. This form will be sent from TLC to qualifying customers in March 2008. Be sure to fill out and return this form to TLC so TLC can contact you to make your travel arrangements.

How is the offer different for Individual and Family plan myFaves customers?

If you have an Individual myFaves plan, you will get a maximum of one booking request form per account, while myFaves for Families customers will get a maximum of two forms per account.

Is there any limit to how many people in a household can participate in the promotion?

The promotion is limited to one booking request form per account for single line plans, and two per account for myFaves for families plans, but there is no limit on how many people in a household are eligible to activate a myFaves plan to receive a free flight. However, other than 2 free flights provided under the same myFaves for families plan, additional members of the same household cannot use free flight booking request forms to fly on the same flights.

Is the Free Flight Booking Request form transferable?

Yes, you can give the booking request form to a friend or family member. But it is not transferable once it has been sent back to TLC with the requested travel dates and destination options. Please remember, the form can only be used by someone over the age of 18 and cannot be sold, traded or otherwise transferred for value.

When will I get the Free Flight booking request form?

If you have met all the eligibility criteria, the Free Flight booking request form will be mailed to you in March 2008. It will be sent to the billing address on the account.

How long is the booking request form valid?

Your booking request form will be valid for travel through March 2009. Keep in mind that 30 days advance notice is required for booking.

I've met all the qualifications. Now how do I book my flight?

Yes. Existing T-Mobile customers can participate in this offer. To become eligible for the Free Flight Booking Request form they must:

* In March 2008, you will receive a free flight booking request form in the mail with instructions for requesting travel dates and destinations if you have met all qualifications.
* Mail the completed booking request form with top three travel choices to TLC for processing and confirmation.
* TLC will contact you within two weeks confirm receipt and will contract you prior to your requested travel dates to book flights and confirm travel options.
* TLC will then send you confirmation of itinerary.
* Booking request forms are valid for travel as early as April 2008 and through March 2009. Travel must be booked 30 days in advance.

Is this offer open to existing T-Mobile customers?

Yes. Existing T-Mobile customers can participate in this offer. To become eligible for the Free Flight Booking Request form they must:

* Switch to a qualifying myFaves plan with a new two-year contract or renew your current qualifying myFaves plan by signing up for a new two-year contract during November 23 to November 25; please contact Customer Care to do this
* Send in the completed application form postmarked by December 31, 2007
* Ensure your account is active on a qualifying myFaves rate plan with no overdue balances as of February 29, 2008.

Are there any fees I should expect to pay?

A $25 booking fee will be charged by TLC. Plus, you will be responsible for any additional taxes or fees charged by the airline, airport or government.

How and when do customers pay for booking fee and additional airline fees and taxes?

Any additional fees and taxes charged by the airline, airport or government are payable at time of booking, when you are making your reservations with TLC. TLC will take payment over the phone via debit or credit card.

Will I be eligible to collect mileage for the free flight if I am signed up with the airline's mileage program?

Yes, you will be eligible to collect mileage in accordance with the rules of the airline's mileage program.

What if my account was post-dated and is set to activate during the November 23 to 25 time period?

To qualify for the promotion, you must initiate the order for a 2 year myFaves contract during the three day period. Accounts ordered prior to the 23rd are not eligible unless they are renewed sometime during November 23rd - 25th, 2007.

Hawaii is not included as either a destination or departure city. Can customers in Hawaii participate in this promotion?

* Yes, eligible customers in Hawaii can participate in this promotion, but they must travel between one of the 58 departure locations and 10 destination locations.
* Eligible customers from Hawaii who use their free flights on the mainland are responsible for booking and paying for their own travel to and from the mainland.
* Eligible customers may also gift their booking request form to a friend or family member to use to depart from one of the 58 departure cities to one of the destination cities. The person receiving the gifted booking request from would then fill it out and submit it to TLC. Free flights cannot be given to someone else once the Booking Request Form is submitted, and cannot be sold or otherwise traded for value.

Are there any travel restrictions when requesting free flights?

* Flights must leave from one of the 58 departure cities and arrive at one of the 10 destination cities.
* A minimum one night stay is required.
* Maximum time between originating and return flight is 30 days.
* Other restrictions based on available airlines schedules and policies also apply.
* Flights must be booked 30 days in advance.
* Must be 18 years of age or older at time of travel to use the voucher.
* Other than 2 free flights provided under the same myFaves for families plan, members of the same household cannot use free flight booking request forms to fly on the same flights. TLC may be able to make arrangements for paying customers to fly on the same flights as members of their household who are traveling using booking request forms.

Once you have booked your travel through TLC, any cancellations or changes are ruled by the airlines' policies. You should contact the airlines directly with any questions regarding cancellations or changes.
