Most shows will feature several writers and directors. All the writers get together to come up with the stories and plots. Then, one writer will be given the job of writing the script for that particular episode. Sometimes they all come up with a broad outline and that writer takes it from there or the writer provides the structure. Either way, they'll come back with the script and that's when they'll re-write either as a group (like on sitcoms where they can try out tons of jokes) or Vince or whoever the creator/showrunner will give their say on the script.
