1. You made a purchase using a debit or credit card between December 4, 2006 and August 10, 2007 at an Olive Garden restaurant in the United States.
2. No other member of your household has submitted a Voucher Claim Form in this lawsuit; and,
3. You are eighteen (1 years of age or older.
