What is the rarest occurrence in the history of the MLB?

I'm gonna go with a pitcher pitching a 3-pitch inning.

We talking single game? Season long?

I took it as single game and that 1 person triple play is pretty rare. I think its only been done twice.
 
I love Met fans who depend on the Yankees losing to be happy.  Also the ones who seem to forget that AJ was on as many teams that one championships and has one more no hitter than the entire Mets franchise history.

I would say four strikeouts in an inning on 12 pitches.  Has that ever been done before?
