Best website to make custom shirts?

My friends birthday is coming up, and we want to make him a custom shirt. 
Any website reccomendations?

Just an image on the front, with a word underneath it. And some other words on the back, kind of like how athletes have their names on the back.

Plain white shirt, if it makes a difference.

choiceshirts.com has a nice interface but have never ordered from them.

makeaball.com can make custom sports balls with you pics, pretty dope if you are looking for that kind of thing.
