The only live one I ever saw was at Doug Benson, and they weren't really hecklers, it was a group (sitting literally front and center, up against the stage) that was finishing his jokes for him. And he didn't directly call them out, but he'd start telling a joke, and then stop before the punchline, and you could hear on of these idiots deliver the rest. And Doug, in his super high drawl, finally said "I don't need to finish that one either." Manager came over and shut them up right quick. That's what I love about comedy clubs, they'll give you the boot.
